Please let me know if you have been traveling in high COVID19 hot spots within the past 5 days of your appointment.

Please let me know if you are currently experiencing any fever, sniffles, cough, sore throat. 

MASKS-are not currently required if you meet the criteria above, but you certainly have that option if you are more comfortable. If you'd like me to wear a mask, I will gladly oblige. Masks may be required in the future depending on the CDC recommendations.

LOBBY-

Please come alone. The lobby will be closed. If you get a ride, I apologize, but your friend cannot come inside. Please bring your own water. The water cooler will be closed.
